We require a valid referral to be sent to the clinic before we can book an appointment. Providers will review their referrals prior to booking an appointment, so our reception staff will give you a call to book an appointment.

Please bring the following:
o Medicare/DVA card
o Referral letter (if we do not already have a copy)
o Any pension, health care or concession cards you may hold
o Any relevant results for the practitioners to review
Payment is required at the time of your consultation.
Please advise the office prior to your consultation if another
party such as WorkCover or an insurance company will be
paying your consultation fees. Before seeing our providers,
we will need your claim number and insurer contact details
to confirm with the third party that they accept to pay your account.
For Your Appointment
It is your responsibility to ensure that you have a valid referral for this appointment. If we do not receive a referral from your GP or Specialist, you will not be able to claim a rebate through Medicare and will be required to pay the full consultation fee.
Please provide 48 hours’ notice if you need to reschedule or cancel your appointment. Failure to do so will result in a $50 cancellation fee.
Reports and Forms
Insurance, legal, NDIS and similar reports are time-intensive and attract a fee. We charge based on WorkCover rates however, individual fees vary. A standard short report is $138, a medium report is $415 and a comprehensive report is $688. Payment is required in full at the time of request.
