o Medicare/DVA card o Referral letter (if we do not already have a copy) o Any pension, health care or concession cards you may hold o Any relevant results for the practitioners to review Payment Information Payment is required at the time of your consultation. Please advise our admin team prior to your consultation if another party such as WorkCover or an insurance company will be paying your consultation fees. |
For Your Appointment
It is your responsibility to ensure that you have a valid referral for this appointment. If we do not receive a referral from your GP or Specialist, you will not be able to claim a rebate through Medicare and will be required to pay the full consultation fee. Cancellation Fee Please provide 48 hours’ notice if you need to reschedule or cancel your appointment. Failure to do so will result in a $50 cancellation fee. |
